The administrator of the policy has no control over the insurance company’s acceptance or lawful denial of anyone to the program. Open enrollment is scheduled for January 1 st of each year, unless otherwise specified or already covered by another insurance program that can be transferred.

Life Insurance

Eligible full-time and part-time employees may participate in the Clinic’s life insurance program. Employees with one year of service are eligible to participate in the Family Life Insurance Term Protection program. Community Animal Clinic will pay up to $40 per month for term life protection. If the premium exceeds $40 per month, the balance is the responsibility of the policy holder (i.e., the employee).

If an employee is no longer employed by Community Animal Clinic, the payment of the policy becomes the responsibility of the policy holder.

Retirement

Eligible full-time and part-time employees may participate in the Clinic’s retirement plan. Employees whose wages exceed $5,000, and have made $5,000 for the past two years, are eligible to participate in the Clinic’s retirement plan. Community Animal Clinic will match up to 3% of the employee’s wages / salary invested.

Employees who do not participate in the plan are not eligible for any contribution by the Clinic.

Each employee will receive information about the plan in October of each year.

Benefits operate on January 1 through December 31 of each year.

Supplemental Insurance

Full-time employees may access a cafeteria plan sponsored by AFLAC through which they may choose from a menu of additional insurance benefits. This offering is made available at 100%
