OUR SERVICES INCLUDE:

• Primary care services for patients across their lifespan, from newborns to the elderly • Treatment of acute and chronic illnesses • Treatment of injuries • Preventive care, including physical exams and cancer screenings • Women’s health/gynecological care • Biopsies, wound care and lesion removals • Behavioral health support and counseling • Substance use treatment and recovery services • Dental care, including preventive and restorative services and dentures • Access to affordable prescribed medications • School, sports, and employment physicals

We also provide supportive services to assist those with exceptional barriers to the receipt of health care services: • Interpretation services – 345 languages available, many in video format • Transportation to and from appointments • Care Coordination, to assist with referrals to outside specialists and other agencies • In-home visits for homebound patients • Food security programs • Support for new mothers, including lactation counseling
